What the page demonstrates
A patterned PDCLC film on a wristband provides a QR-code link to this personal service card. The page illustrates the information that could be accessed during a rehabilitation visit: a participant profile, a visit plan, guidance and follow-up notes.
Material and webpage have different roles
The proposed thermal sequence controls the visibility of the QR pattern on the material. This website is a static information destination. It does not detect temperature, confirm thermal activation, authenticate a wearer or detect when the wristband is removed.
Resetting the material conceals the optical pattern; it does not revoke an already saved link or close an open webpage.
No patient records
This demonstration has no account form, health-data collection, booking service or analytics. Its tabs and expandable sections run locally in the browser.
